Carbon Monoxide Poisoning

A Preventable Public Health Threat

Carbon monoxide (CO) poisoning remains a significant yet preventable public health issue worldwide. Every year, individuals and families lose their lives due to exposure to this toxic gas, often in circumstances that could have been avoided through greater awareness, safer practices, and improved access to clean energy technologies.

 

Recent fatal incidents reported across homes, hotels, workplaces, and recreational settings demonstrate that carbon monoxide poisoning continues to occur in countries of all income levels. While the circumstances vary, these incidents share a common characteristic: exposure to carbon monoxide generated by the incomplete combustion of fuels in enclosed or poorly ventilated spaces.

According to the World Health Organization (WHO), carbon monoxide is one of the major air pollutants of public health concern. It is produced by the incomplete combustion of carbon-containing fuels and contributes to both household and ambient air pollution. Common sources include household cooking and heating appliances, charcoal, coal, kerosene, biomass fuels, motor vehicles, portable generators, industrial combustion processes and wildfires.

Carbon monoxide is a colourless, odourless and tasteless gas that cannot be detected by human senses. Because it provides no warning of its presence, exposure may occur without individuals realizing they are breathing hazardous concentrations.

Carbon monoxide is released whenever fuels such as wood, charcoal, coal, gasoline, diesel, natural gas or kerosene burn without sufficient oxygen. Poor ventilation allows the gas to accumulate indoors, substantially increasing the risk of poisoning.

Why Carbon Monoxide Is Dangerous

The health risk associated with carbon monoxide arises from its ability to reduce the body's capacity to transport oxygen.

After inhalation, carbon monoxide enters the bloodstream and binds with haemoglobin, reducing the amount of oxygen available to vital organs and tissues. As oxygen delivery decreases, normal organ function becomes impaired. High concentrations or prolonged exposure can rapidly become life-threatening.

For this reason, WHO includes carbon monoxide among the six major air pollutants addressed in its Global Air Quality Guidelines.

Health Effects of Carbon Monoxide Exposure

The severity of carbon monoxide poisoning depends on both the concentration of carbon monoxide and the duration of exposure. Early symptoms are often non-specific and may be mistaken for other common illnesses, making carbon monoxide poisoning difficult to recognise.

Common symptoms include:

  • Headache

  • Dizziness

  • Weakness

  • Nausea

 

With increasing exposure, more serious health effects may occur, including:

  • Confusion

  • Loss of consciousness

  • Cardiovascular complications

  • Death

 

Beyond acute poisoning, the World Health Organization (WHO) has identified several health outcomes associated with carbon monoxide exposure. The strength of evidence varies depending on the health outcome:

Sufficient Evidence of a Causal Relationship

  • Reduced exercise tolerance and increased symptoms of ischaemic heart disease following acute exposure (e.g. ST-segment changes)

 

Sufficient Evidence of a Relationship

  • Cardiovascular morbidity, including:

    • Heart attack

    • Congestive heart failure

    • Ischaemic heart disease

  • Short-term exposure associated with hospital admissions or emergency department visits for respiratory conditions

 

Limited or Suggestive Evidence of a Relationship

  • Total mortality

  • Increased risk of cardiovascular mortality and stroke

  • Respiratory conditions, including:

    • Asthma

    • Bronchiolitis

    • Sinusitis

    • Tuberculosis

    • Pneumonia

  • Neurological, neuropsychological, and psychiatric deficits

  • Effects on the developing auditory system

  • Immunological impairment

  • Adverse pregnancy and infant outcomes, including:

    • Low birth weight

    • Congenital defects

    • Infant mortality

As carbon monoxide is colourless and odourless, people may continue breathing dangerous concentrations without realising they are being exposed. Prompt recognition of symptoms and immediate removal from the source of exposure are essential to reduce the risk of serious health consequences.

Carbon Monoxide and Household Air Pollution

Carbon monoxide is one of several harmful pollutants produced by household fuel combustion. WHO estimates that approximately 2.1 billion people continue to rely on polluting fuels and inefficient technologies for cooking, heating or lighting. The use of wood, charcoal, coal, kerosene and agricultural residues generates a complex mixture of health-damaging pollutants, including carbon monoxide and fine particulate matter.

 

Household air pollution was responsible for an estimated 2.9 million deaths in 2021, while the combined effects of household and ambient air pollution are associated with 6.7 million premature deaths annually. These figures highlight the importance of transitioning to cleaner household energy solutions and reducing exposure to combustion-related pollutants.

Preventing Carbon Monoxide Poisoning

Carbon monoxide poisoning is largely preventable. WHO recommends reducing exposure by promoting access to clean household energy, replacing polluting fuels and technologies with cleaner alternatives, improving ventilation, and reducing emissions from combustion sources. Cleaner fuels such as electricity, solar energy, liquefied petroleum gas (LPG), natural gas, alcohol fuels and biogas can substantially reduce exposure to carbon monoxide and other harmful pollutants.

 

Public awareness also plays a critical role. Many carbon monoxide poisoning incidents occur because combustion devices intended for outdoor use, such as charcoal or portable generators, are operated inside enclosed or poorly ventilated spaces.

Carbon Monoxide Beyond Poisoning:
An Air Pollutant of Public Health Concern

Carbon monoxide (CO) is widely recognised as a toxic gas that cause of poisoning in enclosed or poorly ventilated spaces. However, it is also an ambient (outdoor) air pollutant produced through the incomplete combustion of carbon-containing fuels.

Common outdoor sources include:

  • Motor vehicle emissions

  • Industrial activities

  • Power generation

  • Residential fuel combustion

  • Open burning

  • Wildfires

 

Unlike carbon monoxide poisoning, which typically results from exposure to very high concentrations in confined environments, ambient carbon monoxide exposure generally occurs at much lower concentrations over broader populations. Although the health risks differ, both highlight the importance of reducing emissions from combustion sources and improving air quality.

The World Health Organization (WHO) includes carbon monoxide in its Air Quality Guidelines and recommends a 24-hour average guideline value of 4 mg/m³ to help protect public health. These guidelines provide an evidence-based reference for countries when developing and reviewing national ambient air quality standards.

Cleaner Air Starts with Prevention

Reducing carbon monoxide exposure requires action at both the individual and community levels. Preventing poisoning through safe use of fuel-burning appliances, improving ventilation, reducing emissions from combustion sources, and strengthening public awareness all contribute to healthier environments and cleaner air.

Despite advances in technology, safety regulations, and public awareness, carbon monoxide exposure continues to result in preventable illness and death across the world. Reported incidents occur in homes, hotels, restaurants, workplaces, vehicles, temporary shelters, and other enclosed environments where carbon monoxide can accumulate due to incomplete combustion and inadequate ventilation.
